Long considered a field dominated by descriptive phenomenology, it was the discovery of dopamine deficiency in Parkinson's disease and the efficacy of levodopa therapy which finally allowed this field to emerge as a truly therapeutic discipline. This discovery of a selected neurotransmitter deficiency linked to a specific pathologic lesion in Parkinson's disease, and the remarkable therapeutic response when the deficiency was corrected served as the driving force in focusing active research into the function of various areas of the brain, most notably the basal ganglia. In addition, the motor complications seen with dopaminergic therapy in Parkinson's disease and those occurring with antidopaminergic therapy in the treatment of psychiatric disorders have also played a critical role in the understanding of the phenomenology and pathophysiology of movement disorders. These developments have so stimulated the interest of neuroscientists that movement disorders presently represent one of the most active areas of research in all of neurology and psychiatry. Most national and international meetings now have separate sessions for presentations dealing with movement disorders where there are often more papers than in any other single area. A new international society and video-tape journal have recently been established for the study of movement disorders and special interest clinics are being opened at most major medical centers. The field includes many subspecialty disciplines. Clinical phenomenology remains quite important as indicated by the great interest in video tape demonstrations of unusual or difficult cases. The disciplines of genetics and molecular biology, epidemiology, toxicology, pharmacology, biochemistry, physiology, and pathology all play a role in this expanding field.
